A client intake form is the structured questionnaire a new client fills out before their first service. Depending on the business it captures contact details, allergies and sensitivities, medications, past treatments, skin or hair history, goals, and signed consent or waivers. For a med spa, esthetician, or waxing studio it is also a safety and liability document; for a hair or nail salon it is the foundation of personalization. Either way it turns an anonymous walk-in into a known client with a record.
Intake matters operationally because it front-loads the friction. A form completed online before arrival means less paperwork at the chair, a provider who already knows the contraindications, and a cleaner start to the appointment. It also feeds the client record that powers everything after: reminders, rebooking, and the marketing that keeps someone coming back. Collected at booking, it even helps reduce No-Show Rate, because a client who has invested a few minutes in a form and left a card on file is more committed to showing up.
